Donyell Malen gave the Netherlands the lead in the 34th minute, but Andrej Kramaric equalized from the penalty spot in the 55th.
Luka Modric put Croatia ahead in the 72nd minute, but Steven Bergwijn equalized for the Netherlands in the 89th minute to force extra time.
Bruno Petkovic scored the winning goal for Croatia in extra time, curling home a shot from the edge of the penalty area.
The win was a major upset for the Netherlands, who were unbeaten in their previous 11 matches.
Croatia will be looking to win their first major trophy since the 2018 World Cup. They will face a tough test in the final, but they will be confident of their chances after their impressive performance against the Netherlands.
